Kindergarten Pre-Registration
Parents should
take the following items with them when they go to the school to
pre-register their child.
PROOF OF
RESIDENCY The parent will be asked to
provide a current utility bill and a lease agreement, contract on a
home, warranty deed statement, or mortgage statement. The school
will accept a current gas, electric, or water and sewer bill.
Telephone and television cable bills will not be accepted.
LEGAL BIRTH
This must be the certificate issued BY THE
STATE
CERTIFICATE OR COUNTY where the child was born – NOT THE HOSPITAL
RECORD. He/she must be five (5) years old on or before September 1,
2005.
IMMUNIZATION
The State of Oklahoma requires *acceptable evidence
RECORD of adequate immunization before a child may be enrolled in
any school in Oklahoma. Minimum immunizations required by state law
for students entering kindergarten for the first time are:
1. 5 doses DPT- if the fourth DTP or DTaP was received after the
child’s fourth birthday, the fifth dose will not be required.
2. 4 doses of Polio - if the third dose was received after the
fourth birthday, the fourth dose will not be required.
3. **2 doses of MMR (measles, mumps, rubella) - one dose of MMR must
have been given on or after the child’s first birthday, 2nd dose may
be just measles or MMR.

4. 2 doses of Hepatitis A - there must have been six months between
the 1st and 2nddoses.
5. 3 doses of Hepatitis B
6. **1 dose of Varicella (chicken pox) or can be verified by
history.
*Acceptable evidence of adequate immunizations: “Records provided by
a licensed physician or public health authority that clearly
indicates which immunizations have been received, the dates they
were administered, and the signature or stamp of the physician or
health clinic that administered the immunizations or interpreted the
immunization history.” Oklahoma School Immunization Law
**A previous history of having a vaccine preventable disease is not
generally considered grounds for waiving the required immunization.
However, measles immunization may be waived if the parent or
guardian presents a signed statement from a licensed physician that
indicates the child has had the disease. The Varicella vaccine may
be waived if the parent or guardian presents a signed statement that
indicates the child has had the disease. A history of having rubella
is not grounds for waiving the immunization.
Parents who have applied for a transfer to a
school other than their neighborhood school should pre-register at
their neighborhood school unless they have been notified that their
transfer request has been granted.
